Collaboration Manager failed to create a workspace with "Error occured". Looking at the ULS logs you discover an error like:

This could be caused by an iisreset or service start/stop operation while the user profile service unloads the profile but couldn't succeed.
Restart the server and / or make sure the following Group Policy setting has been set at Machine/Computer Configuration/Administrative Templates/System/User Profiles
